Needed Forms for Temporary/Student Temporary/PERF
- US Temporary Employees Require the Following Forms –
- Kelley School of Business Temporary Appointment Form
- Indiana University – Personal Profile Form (ED)
- Indiana University Background Check Consent Statement
All new temporary/student temporary employees are required to have a background check. - Indiana University Employment Application Form: HTML | PDF
- I-9: As soon as a temporary employee is hired, please send Kaylin Boyd (kayrboyd@iu.edu) an e-mail with the new hire's first and last name, start date, and e-mail address (IU e-mail is not required) so that the hiring process can be initiated. The new hire will receive an e-mail from HireRight and will need to fill out Part I of the I9. After completion, the new hire will need to bring correct identification for the I9 form to Kaylin in the Business Office (CG 3023). The acceptable ID documentation types will be listed in HireRight.
- Once all the paperwork has been turned in and entered, employees will be sent tax forms and direct deposit via email.

- International Tax Questionaire
This replaces the W-4 & W-4H for international employees - Once all the paperwork has been turned in and entered, employees will be sent tax forms and direct deposit via email.
The student will also need to submit the following documents to Kaylin Boyd so that they may be copied and the I9 completed:
- Passport – US Visa Page and the inside page with the student’s picture and foreign information
- I94 – This is the white card that is in their passport and it is stamped when they enter the country
- Social Security Card or the letter from the social security office stating that they have applied for a card
- I-20 – This is for students who are on a F-1 Visa
- DS-2019/IAP-66 – This is for students who are on a J-1 Visa
